Editable Website Widget Forms

Clinic users can select and edit the forms displayed in the Website Widget that are linked to each request option.

Any form linked to a request type that is enabled for clinics on the Settings > Website Widget page will be visible on the Forms page with the Website Widget label.

Clinics will have the ability to edit Website Widget form as needed within the forms screen.

If a clinic user makes a change on the form, the change will be automatically reflected on their website (they’ll need to refresh their website page first to see the changes).

Forms associated with a Website Widget request cannot be deleted. You must choose a different form for the request type(s) or delete them before the form can be deleted.

If a clinic user adds a new request type on the Settings>Website Widget page, they can choose an existing form or select 'Default' which will create a new form based on the request type. From there, the clinic user will have the ability to edit the associated form on the Forms page.
