To get started with editing forms:
- To edit current forms, navigate to Forms in the menu in the lower-left corner of Flow.
- All forms will be listed on the left side of the page or found in Archived section
- Forms in the Archived section can be re-activated or deleted
- There are many possible ways to edit a form once it is open:

New items can be added anywhere in the form by clicking the button that says +Add New Question

Items can be removed by clicking the garbage can icon

Items can be reordered by clicking the arrows up or down (next to the garbage can icon)

The text presented to clients can be edited
- As soon as you make your first change to the form, an option will appear at the bottom of the screen to save your unsaved changes. Don’t forget to save your work!

💡 Helpful Tip: Click on the wildcard icon to autofill information like the pet’s name or client’s name
Compose Mode Glossary of Terms
There are several types of form items that you can use. They are listed below so that you can better know which one is right for you and what your clinic is trying to do with a specific form.
- Text Input - This will ask the client for an open-ended input that they can type into. This is a common catch-all on forms to ask for any additional information or symptoms that weren’t described anywhere else
- Multi Line Text Input - This will ask the client for an open-ended input that they can type multiple lines into. This is commonly used when asking clients to list things such as current medications for a pet
- Multiple Selection - This will present the client multiple options, of which they can select as many as they would like. This is commonly used when asking clients for a preferred day of the week to schedule an appointment, or to select from a list of symptoms that their animal may be experiencing
- Drop Down - This will present the client with multiple options, of which they must choose one. This is commonly used when asking clients about a pet’s species or sex
- Checkbox - This will present the client with a checkbox that they must acknowledge before moving on. This is commonly used for ensuring clients read important instructions
- Date Input - This will ask the client to either enter a date by typing or choose one from a calendar. This is commonly used for asking for a pet’s birthday or date of adoption
- Time Input - This will ask the client to enter a time by typing or choose a time from a drop-down menu
- Text Block - This requires no input from the client and will instead show them the text that you enter. This is commonly used to break up forms into sections or present information that a client needs to know ahead of an appointment
- Signature Input - This will ask the client to both type their name and sign their name digitally. This is commonly used on consent forms ahead of anesthetic usage or client codes of conduct
- Rating - This will ask the client for a rating out of five starts. This is commonly used in post-appointment follow ups asking for feedback on the appointment
